Description: Successfully completes the goals and objectives established for the residency program and those specific to each rotation. Satisfactorily completes one major project during the residency program (1 year) and other projects assigned while on rotations. Provides a safe and effective drug control system; procures, compounds, prepares and dispenses pharmaceuticals according to policy and procedure. Provides medication therapy management care to the patient populations served; identifies, prevents and resolves medication related problems to promote safe and effective use of medications and to facilitate positive patient outcomes; documents (as required) the appropriate findings, interventions, recommendations, plan and/or outcomes. Provides education/consultative services to patients and professional staff to assure appropriate use of medications. Provides functional supervision to the pharmacy operations in assigned work area. This includes responsibility and accountability for ensuring timely, accurate completion of all pharmacy functions, duties, and responsibilities during assigned work period/shift Demonstrates commitment to the pharmacy team provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments. (if applicable) Maintains required department confidentiality and abides by HIPAA regulations. Takes an active role in enhancing ability to carry out job functions through personal and professional/job-related growth and development, and participates in medical center and/or departmental education programs. Supports and abides by all medical center, departmental, and safety policies and procedures. Proactively identifies safety concerns surrounding the work environment. Performs other responsibilities as requested by supervisor, manager or department head.
ADDITIONAL FUNCTIONS
Provides formal in-service/teaching to students, pharmacy staff and other health care professionals. Participates in quality improvement activities; e.g. process improvement teams, DUE/DUR, development of protocols, etc. Participate on committees; e.g., Pharmacy and Therapeutics, others as identified. Participates in departmental visitations and resident recruitment. Maintains records as required by Federal and State Law. POSITION QUALIFICATIONS
Education formal : Pharm D Pharmacy Degree is required Iowa Pharmacist Licensure or eligible for licensure, either by examination or reciprocity is required. Must obtain licensure within 60 days of hire.
Experience:
1. Previous practical pharmacy experience is preferred.
Special Skills and Competencies: Self-motivated, detail-oriented individual able to work with minimum supervision. Supervisory and planning abilities. Good written and oral communication skills Ability to work on multiple projects at one time and flexibility to meet department needs. Completes the following mandatory courses (check all that apply).
