Reports to Permanent Supportive Housing Supervisor
Position Status Full-Time
Shift Monday - Friday, 40 hours
Grade, Starting Rate Grade 6, $18.90/hour
FLSA Status Non-Exempt
Housing First - Provide outreach services to program participants in their homes, emergency shelter, and other locations.
- Assist guests in identifying barriers and connect them with appropriate resources in the community.
- Strong belief of the Housing First model.
- The ability to engage and encourage housing conversations with guests.
Managing a Case Load - Meet with families in their homes to assess needs and work to establish goals with the family
- Driving is required for this position and having reliable transportation is preferred.
- Assist family in obtaining income through employment or other benefits and Focus on housing and strengths-based practices to meet the unique needs of each family.
- Assess children's needs to make appropriate community referrals and liaise with schools as needed for supportive services
- Daily collaboration with all departments promoting a healthy team environment.
- Seeking out, engaging, and networking with community partners to find housing options and additional resources.
- Attended weekly team meetings and contribute to case conferencing conversations.
- Maintain accurate records, case management notes, and statistical data as required and participate in special projects as needed.
Resource Collaboration - Provide direct, trauma-informed case management services to families, including outreach, ongoing assessments, goal setting, creative problem solving, using a network of community resources.
- Collaborate with community partners and agencies to support families experiencing mental and physical health needs.
- Ability to accept direction and constructive feedback from Supervisor.
Education and Experience - Experience working with a diverse population preferred
Physical and Equipment Requirements - Ability to lift 25 pounds.
- Ability to stand and/or walk for at least an hour at a time.
- Ability and willingness to drive an agency vehicle.
