Requirements:

• Active CA License

• BC/BE

• BLS, PALS

• Active DEA

• PAs must have CA Controlled Substance Educ Course Cert

• NPs must have Furnishing License w/ Schedules II

• Immunizations such as Flu Vaccine, TB Status/X-Ray Results

• COVID vaccination proof (Primary series + booster, regardless of the date received OR 1 dose of current COVID vaccine (2023/2024 formulation)

• At least 3 yrs of experience in similar setting
