Join BJC Home Care providing pediatric case management for home care, home infusion and palliative care. This is a unique opportunity to provide a lev el of care that is missing in our community working with a small, close knit team. Experience with pediatrics nursing highly preferred. Schedule Monday Friday 8 a.m. 4:30 p.m. with on call, weekend and holiday rotation. Perks Mileage reimbursement at the current IRS rate. Location Home Base Mason Ridge Center Drive in Chesterfield

Overview

BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.
BJC Home Care provides care to thousands of patients in both Missouri and Illinois. The BJC Home Care Services Supportive Care staff offer patients with life limiting illness an opportunity to ease the transition from curative treatment to Hospice care through a unique program. Supportive Care offers a bridge between curative and terminal care. It is a multidisciplinary approach, holistic in nature, centered on the needs of the patient and patient's family, that provides coordinated medical, emotional and spiritual care.
Preferred Qualifications

Role Purpose Coordinates, negotiates, procures and manages the care of patients to facilitate achievement of quality cost effective outcomes. Works collaboratively with physicians and interdisciplinary staff, internal and external to the organization, to identify and resolve issues. Performs and provides consultation regarding Utilization Management and Performance Improvement programs in order to obtain optimum value for the patient and the reimbursement source. Responsibilities
•  Assesses, coordinates, and evaluates each patient's plan of care.
•  Establishes, continuously evaluates, and documents an individualized discharge plan.
•  Ensures clinical documentation and cost effective use of hospital resources. Minimum Requirements Education
•  Nursing Diploma/Associate's Nursing Experience
•   Supervisor Experience
•  No Experience Licenses & Certifications
•  Valid Driver's License
•  RN Preferred Requirements Education
•  Bachelor's Degree Nursing Experience
•  2 5 years
