POSITION SUMMARY: Manages and coordinates Care Coordination services within the health system, providing person-centered care to patients in various settings. Collaborates with staff to ensure high-quality services and develops protocols with clinical leaders.
REQUIREMENTS:
EDUCATION: Registered Nurse, BSN preferred. CERTIFICATION/LICENSES: Current New Mexico Nursing License. SKILLS: Knowledge of healthcare policies, ability to coordinate services, strong communication skills, and cultural sensitivity. EXPERIENCE: Minimum two years in healthcare, with a focus on quality and care coordination preferred. NATURE OF SUPERVISION: Reports to Manager of Population Health and Director of Ambulatory Patient Care Services. ENVIRONMENT: Various healthcare settings with potential exposure to bloodborne pathogens. PHYSICAL REQUIREMENTS: Mobility in care settings, good vision, dexterity, and excellent customer service skills.
