About the Position:
Group is seeking a dedicated and experienced Nurse Practitioner to join our Adult Palliative Care service and help expand our well-established interdisciplinary program. This role is integrated into a supportive team that includes Hospitalist physicians and Nurse Practitioners, experienced case managers, and a Spiritual Care Director.
Position Highlights:
Full-time, Monday Friday, 8:00 AM 5:00 PM
No call and no weekend responsibilities
Focused on inpatient palliative care no outpatient duties
Perform comprehensive assessments and manage symptoms/medications for hospitalized patients
Provide consultations across various specialties, including critical care, oncology, neurology, cardiology, and general medicine/surgery
Collaborate closely with referring providers to develop and implement patient-centered care plans
Lead goals-of-care discussions with patients and families to ensure treatment aligns with their preferences
