Duties: Family Practice
Days per month: 20
Required Procedures: toenail removals, lesion removals, Pap Smears, Joint injections. (negotiable)

Support Staff: Medical Assistant/Triage RN/Front Office Staff/Clinic Manager
Reason for Coverage: Provider retirement
Travel Preferences (see attachment for specific travel guidelines):
Preferred Lodging: none. In the event that the Facility preferred hotel is not available, hotel/lodging rates will be reimbursed based on actual expense, up to 1.5 times the current GSA rate for the market.
Air Travel: $750/round trip cap
Rental Cars: $65/day all in cap (including taxes and fees) regardless of car class.
Mileage: IRS Standard Rate
