We currently have an exciting opportunity for an Onsite IP Clinical Pharmacist to work full-time at St Bartholomew's Practice within Oxford PCN. This role is suitable for a highly motivated and focused Clinical Pharmacist-IP, responsible for managing day-to-day activities within general practice, providing pharmaceutical care and advice to patients, and adhering to safety guidelines.
Full-time hours are within the core opening hours of the GP practice.
Main duties of the job
As an onsite Clinical Pharmacist-IP, you will confidently operate within professional boundaries, supporting and collaborating with a team of pharmacists and pharmacy technicians.
Your responsibilities include resolving medication-related queries, working as part of a multidisciplinary team, improving patient outcomes, and enhancing access to healthcare.

Job responsibilities
You will support and work alongside pharmacists and GPs, confidently acting within professional boundaries.
- Conduct polypharmacy medication reviews and SMRs
- Perform face-to-face medication reviews
- Utilize skill mix effectively
- Improve healthcare access
- Assist practice staff with prescription and medication queries
- Carry out medicine reconciliation and post-discharge reviews
- Conduct medical audits and contribute to service improvement
- Provide expert medication advice to clinicians and patients
- Lead on medicines optimisation and quality improvement initiatives
- Ensure the appropriate and efficient use of medications
- Educate patients on medication use and disease prevention
- Understand QOF, CQC, and local incentive schemes
- Create and follow clinical pharmacy programmes according to practice policy
Person Specification Qualifications
- Independent Prescribing Qualification
- CPPE Pathway or Exemption
Experience
- 6-12+ months in primary care or GP practice setting
