- Board Certified Only: REQUIRED
- Active WA License or IMLC: REQUIRED
- Certifications BLS: REQUIRED

- 100% 18 + population
- Patients per day: 15
- Percent of Practice: Office/Clinic: 99%, Hospital: 1%
- New Consults per week: 5
- Number of Oncologist in Practice: 1 RadOnc, 2 MedOnc,
- Number of other healthcare professionals in practice: 1 Dosimetrists, 1 Physicist, 4 Radiation Therapists
- Is locum tenen provider required to supervise APP's? No
- X-Ray Onsite
- RN's, MA's RN Navigators, Social Worker, Occupational Therapist
- Treatment Plan Computer System:
- Office Equipment: Varian IX, Varian TruBeam, HDR Machine, GE Wide Boar CT Scanner
- EMR: Epic
