POSITION SUMMARY: We have an exciting opportunity to join our team as a Licensed Practical Nurse-NYU Langone Medical Associates- Riverhead. In this role, the successful candidate Responsible for using own clinical experience to plan the delivery of safe, therapeutic, & age appropriate patient care via planning, implementation & evaluation over a scope of practice. With the patient and family central to the care, the LPN incorporates the unique characteristics and needs of each patient and family member into the care delivery process. Works collaboratively within a multidisciplinary Health team to promote wellness and optimizes patient outcomes. Performs multiple clinical and clerical duties as required. Complies with established policies, procedures, rules and regulations as they pertain to the Hospital as well as federal state and local regulations. Diligently observe all HIPPA guidelines. Performs all other duties as it relates to specialty.
JOB RESPONSIBILITIES:
- Performs other duties as assigned
- Assess and document patient family advance care directives and Rights and Responsibilities if indicated. Establish collaborative and collegial relationships with health care team members.
- Enters comments in system when receiving verbal approval from a physician to refill a prescription.
- Maintains exam room for necessary supplies, cleanliness.
- Obtains referrals for patient tests ordered by MD as per patients insurance requirements.
- Utilizes knowledge of nursing theory, research and practice in the application of clinical judgment.
- Initiates appropriate interventions to amend identified variances from plan of care. Documents patient and family response to interventions and collaborate with the RN to modify the plan of care as needed. Provides direct patient care which includes but not limited to utilizing WUH policy procedures, patient care guidelines, physicians' orders etc. Incorporates standards of practice, clinical pathways, age-specific guidelines and patient care guidelines into plan of care.
- Interacts and collaborates with other members of health team to identify specific patient needs.
- Identifies patient satisfaction dissatisfaction with plan of care. If patient expresses dissatisfaction, implement conflict resolution skills and addresses issues with plan of care.
- Processes medication orders and refills. Verifies dosage and medication using various tools progress notes, clarity, physician or contacting patient if necessary.
- Obtains prior authorization for medication from patients insurance company as needed,
- Recognizes and responds to issues that may cause problems and or refers issues to the RN; utilizes the escalation policy. Recognizes the family as defined by the patient and incorporates the principles of family centered care in daily practice. Identifies patient satisfaction dissatisfaction with plan of care; escalates issues as required. Consistently verifies and documents patient advance care directives for assigned patients. Maintains open channels of communication through intra and inter shift reporting. Utilizes appropriate communication in all interactions; demonstrates proper telephone etiquette. Actively participates in maintaining a clean, safe, therapeutic environment, including security of patient belongings. Consistently complies with regulatory and WUH policies governing safety standards, infection control and patient belongings. Responds to emergency situations as appropriate. Ensures appropriate emergency equipment is present and operational. Is cognizant of HEICS plan and participates in implementing plan as required.
- Evaluates patient progress toward health goals and modifies critical path or plan or care accordingly.
- Completes documentation reflecting multidisciplinary coordination and the teaching learning needs of patients and families while on program in preparation for discharge. Evaluates and documents patients response to nursing and multidisciplinary team interventions, including patient family teaching. Evaluates patients clinical status and attempts satisfactory resolution of patient clinical issues at point of care prior to escalating issues. Takes action to optimize achievement of patient outcomes. Participates in identification, investigation and resolution of actual and or potential risk management quality care ethical issues. Addresses patient issues following established communication hierarchy. Works in collaboration with the primary nurse.
MINIMUM QUALIFICATIONS: To qualify you must have a High School or GED. Technical Diploma Graduate of a licensed practical school of nursing. Current NYS Licensed Practical Nurse. 1 - 2 years Recent medical/surgical Hospital experience or 1 year experience in a Home Health agency. Excellent communication skills (both oral & written), interpersonal, and organization skills. Basic Computer Knowledge. Demonstrates critical thinking skills to problem solve. Evaluate the physical, social, and psychological status of patients of all ages employing interviewing and physical and psychosocial assessment skills REQUIRED LICENSES: LPN
