About the Role We're seeking a passionate and skilled
Occupational Therapist to join our Home Health team. In this role, you'll provide in-home evaluation and therapy for clients with physical, emotional, or cognitive limitations. You'll help patients achieve greater independence through customized care plans, family education, and goal-oriented therapy interventions.
What You'll Do Conduct home-based assessments within 72 hours of referral (unless otherwise specified).
- Design and implement individualized treatment plans focused on improving daily living skills and function.
- Educate patients and caregivers on therapy techniques, home exercise programs, and insurance guidelines.
- Participate in interdisciplinary case conferences and collaborate across care teams.
- Maintain clear, accurate documentation in compliance with all home health guidelines and regulations.
- Ensure care is age-appropriate across the lifespan, from infants to geriatrics.
What You'll Need Bachelor's degree in Occupational Therapy required (Master's preferred).
- Current Iowa Occupational Therapy License (provisional license with supervision accepted for new grads).
- Valid Iowa driver's license, proof of insurance, and reliable transportation.
- BLS certification and Mandatory Reporter certification.
- 0-2 years of experience in occupational therapy (home health experience preferred).
- Strong communication, organization, and independent problem-solving skills.
