Occupational Therapist

Requires a graduate of an accredited program and state license, located at Mulford Clinic in Rockford. Responsibilities include evaluation, treatment planning, documentation, and mentoring. Must demonstrate quality, service, partnering, and cost-saving behaviors. Education: Graduate of an accredited school of Occupational Therapy. Certification/Licensure: Licensed as an occupational therapist in WI, IL or both if required. Additional requirements include passing background checks and following instructions. Work environment is usually quiet. Age groups served: Neonate to Geriatric. Work contact group includes partners, physicians, patients/family, visitors, vendors. Special physical demands are essential job functions. Level of supervision requires some independent judgment.
