Outreach PT, OT and SLP is one of the leaders of in home out patient physical, occupational and speech services in New York and New Jersey. Due to our continued growth , Outreach is looking to bring on additional motivated and passionate Therapists to work with the adult and geriatric populations either in their private home, apartment or in an Assisted Living Facility. FULL TIME OR PER DIEM : MANHATTAN

POSITION SUMMARY In this position, you will perform in-house Occupational Therapy.

The Occupational Therapist is responsible for:
Home safety assessment : assessing environmental barriers and extrinsic risk factors for a fall in individualized environment
Ordering of adaptive equipment
Cognitive and or visual assessment
addressing underlying impairments relating to ADL and IADLs
Educating patient on home program along with caregiver/family members,

Job qualifications
Masters degree a minimum for Occupational Therapist (OT)
1 year of experience working with older adults preferred.
Home care experience a plus.
