Occupational Therapist

•  Split hours: 20% at Creston Home Care, 80% at Mt Ayr Hospice.
•  Provide social work services, psycho-social assessments, and connect patients/families with community resources.
•  Develop and maintain care plans, coordinate with healthcare teams, assist with orientation, and represent agency in the community.
•  Requirements: BSW (MSW preferred), 1 year social work experience, CPR and mandatory reporter certification, valid driver's license, and knowledge of local resources.
•  Skills: Strong communication, adaptability, documentation, teamwork, and cultural competence.
•  Physical requirements: Medium exertion, flexibility in schedule, and travel between sites.
