Facility in Layton, UT seeking ObGyn locums coverage Rates: $2500/day with 4 hour gratis, $350/hr after 4 hours Details: - Coverage Start Date: As soon as credentialed
- Setting: Outpatient, Inpatient, ER, Clinic, Call
- Reason for Coverage: Backfill needed due to the loss of two OB/GYN providers
- EMR: EPIC
- Support Staff: Medical Assistant (MA)
- Required Procedures: Gyn and OB patients, including surgeries and assisting in surgery
- Trauma Level: N/A
- Beds: 220 beds
Schedule/Availability: - Clinic: Monday, Tuesday, Thursday from 8 AM to 5 PM
- Call: 24-hour call shifts (7 AM-7 AM), includes Wednesdays and 2 weekends per month (Friday-Monday)
- Call Back: 0-2 times per shift
- Shifts/Month: 7-10 call shifts per month, clinic schedule is flexible
Requirements: - Board Certified OB/GYN (ABOG): Required
- Active UT License: Required (IMLC with LOQ acceptable)
- BLS: Required (or willingness to obtain)
- DEA: Required
- Shifts Per Month: Must state the number of shifts the candidate can offer
- Clean Malpractice: Required (up to $100k acceptable with explanation)
- NPDB Self-Inquiry: Required
- Clean Background: Required
- EMR Training: Must complete required training modules before starting
Credentialing Timeline: 90 days after receiving a completed application
