What You'll Do in This Role:
The LifeCircles clinical team delivers primary care, palliative care, and end-of-life care for patients aged 55 and older. This position is perfect for the healthcare provider who wants to be patient-centered, creative, and collaborative in their approach to care.
The successful candidate will join an experienced Interdisciplinary Team and work closely with other Advanced Practice Providers and healthcare professionals in a well-supported, team-based environment.
Primary Responsibilities:
Participating in daily interdisciplinary team meetings
Completing periodic health assessments
Assessing and treating acute needs and illness
Advanced Care Planning
Patient and Caregiver Education
What You'll Need:
Current license and certification to practice as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of Michigan
Master's degree in nursing or satisfactory completion of a physician assistant program
BLS certification
Current DEA registration
State of Michigan Controlled Substance License
Minimum of 1-year experience with the frail and elderly or long-term care population preferred
