We are a local PACE program (Providing All Inclusive Care to the Elderly) right here in Greensboro, NC. In this role, you will work as part of our care team, which includes other providers, RNs, CNAs, and Physical Therapists. This position requires an active and unencumbered NP license. Some of the duties of this role include:

  • Attend trainings as needed
  • Perform health histories, physical exams, and teachings
  • Serve as liaison between senior leadership and staff
  • Develop treatment plans for patients
  • Assist with policies and procedures
  • Work with a larger care team
  • Assist with audits as required
  • Maintain current credentials and licenses

Are you a compassionate, dedicated Nurse Practitioner with a passion for providing exceptional care to the geriatric population? We are seeking an experienced Nurse Practitioner to join our team in the PACE (Program of All-Inclusive Care for the Elderly) Program. This is a permanent, full-time position that offers the opportunity to work with a multidisciplinary team focused on providing comprehensive healthcare services to elderly individuals in our community.

This is a full time role, 40 hours a week working a Monday through Friday daylight schedule.

Responsibilities:

As a Nurse Practitioner in our PACE Program, your responsibilities will include:

1. Providing comprehensive healthcare services to PACE participants, including assessment, diagnosis, treatment, and follow-up care.
2. Collaborating with a multidisciplinary team of healthcare professionals to develop and implement individualized care plans for each participant.
3. Educating participants and their families on disease prevention and health maintenance.
4. Conducting regular health assessments to monitor participants' health status and progress.
5. Coordinating with other healthcare providers to ensure seamless, integrated care for participants.
6. Participating in PACE team meetings to discuss participants' care plans and progress.
7. Adhering to all regulatory standards and best practices in geriatric care.
8. Documenting all patient care in accordance with established standards.

Qualifications:

To qualify for this exciting opportunity, you will need:

1. A Master's degree in Nursing from an accredited institution.
2. Current licensure as a Nurse Practitioner in the state of practice.
3. Certification from the American Nurses Credentialing Center (ANCC).
4. A minimum of 2 years of experience as a Nurse Practitioner, with a focus on geriatric care proffered.
5. Demonstrated experience with the PACE program or equivalent.
6. Exceptional clinical skills with the ability to assess, diagnose, and treat a wide range of health conditions in the geriatric population.
7. Excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team.
8. Strong organizational and time management skills, with the ability to manage a high caseload effectively.
9. A commitment to providing compassionate, patient-centered care to the elderly.
