Job Summary
We are seeking an experienced Family Medicine Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in Colebrook, NH. This position involves providing high-quality primary care to individuals and families in a rural health clinic setting. As part of a dedicated healthcare team, you will deliver comprehensive health services, including preventative care, diagnosis, treatment, and management of common and complex health issues.

Key Responsibilities:

  • Provide comprehensive care to patients of all ages within a family medicine practice

  • Perform routine physical exams, preventive health screenings, and immunizations

  • Diagnose and treat acute and chronic medical conditions

  • Prescribe medications and manage treatment plans

  • Collaborate with other healthcare providers to coordinate patient care

  • Educate patients on health maintenance and disease prevention

  • Document all patient encounters in an accurate and timely manner

  • Participate in patient care planning and quality improvement initiatives

  • Adhere to clinic policies, state regulations, and industry standards

  • Provide compassionate, patient-centered care to a diverse community in a rural setting

  • Work collaboratively with physicians, other APPs, and clinical staff

  • Demonstrate a commitment to patient safety and quality care

  • Maintain professionalism and confidentiality in all interactions

  • Provide leadership and mentorship to less experienced staff when needed

  • Display strong communication skills and the ability to connect with patients and the community

Education and Experience Requirements:

  • Graduate from an accredited Nurse Practitioner or Physician Assistant program

  • Active State of New Hampshire NP or PA License

  • Minimum of 2 years of experience in Family Medicine or Primary Care

  • Experience in rural healthcare settings preferred

  • Current CPR certification

  • Strong clinical judgment and the ability to make evidence-based decisions

  • Ability to work independently and as part of a collaborative team
