Your Role
As a Nurse Practitioner in our Palliative Care Team, you will:
- Conduct comprehensive physical and psychosocial assessments of patients.
- Create and implement evidence-based, patient-centered care plans.
- Provide guidance and education to patients and their families on disease management, symptom control, and navigating care transitions.
- Collaborate with a multidisciplinary team, including physicians, nurses, social workers, and chaplains, to ensure seamless, compassionate care.
- Offer expertise in managing symptoms such as pain, breathlessness, nausea, fatigue, and more.
- Serve as a key resource for patients and referring providers, ensuring continuity and quality of care across settings.
This position primarily involves Monday-Friday, 8 AM-5 PM clinic care, giving you the opportunity to provide deeply impactful care while maintaining work-life balance.
About You
We are looking for a team player with excellent communication skills and a passion for delivering patient-focused care. The ideal candidate will:
- Demonstrate a commitment to holistic care and a genuine interest in supporting patients with serious illnesses.
- Be skilled in critical thinking, complex problem-solving, and evidence-based decision-making.
- Have experience in or a strong interest in palliative care, or related specialties.
- Be self-aware and have the capacity for introspection
Required Qualifications
- Education: Graduate of an accredited Nurse Practitioner program.
- Licensure: Current license to practice as an NP in Texas.
- Certification: Current certification by the appropriate board (NCCPA for PAs; AANP/ANCC for NPs).
- Basic Life Support (BLS): Certification within 30 days of hire or transfer.
- Experience: We would prefer someone with some medical experience, but are open to motivated new grads
