Job Overview - Nurse Practitioner (NP) Supervisor:
Compensation: $84/hour
Location: New York, NY
Schedule: Monday to Friday, 8:00 AM - 5:00 PM
Become a key leader with our client in New York, NY, as a Nurse Practitioner (NP) Supervisor in a PACE (Program of All-Inclusive Care for the Elderly) setting. In this high-impact role, you'll supervise NPs, manage care plans for high-risk patients, and coordinate with interdisciplinary teams to deliver comprehensive, patient-centered care across both in-center and home-based environments.
Responsibilities as the Nurse Practitioner (NP) Supervisor:
- Clinical Supervision: Oversee Nurse Practitioners at PACE sites and support clinical operations at the Diagnostic Treatment Center (DTC).
- Leadership & Representation: Serve as delegate to the Medical Director and represent clinical leadership in interdisciplinary team meetings.
- Patient Evaluation & Care Planning: Conduct assessments, manage high-risk patients, and lead care planning across settings.
- Documentation & Risk Management: Ensure accurate clinical documentation and support coding, risk adjustment, and compliance initiatives.
- Education & Family Engagement: Provide education to participants, families, and staff on disease management and wellness strategies.
- Home & Community-Based Care: Conduct home visits, oversee community care plans, and assist with emergency and on-call coverage.
Qualifications for the Nurse Practitioner (NP) Supervisor:
- Education: Graduate of an accredited Nurse Practitioner program recognized by NYSED.
- Certification: National certification by AANP or ANCC required, along with active DEA certification for prescribing authority.
- Licensure: Active Nurse Practitioner license required in the State of New York.
- Experience: Minimum of 3 years of clinical practice experience in home care, care management, geriatrics, internal medicine, or primary care settings.
- Technical Skills: Proficient in Microsoft Office and electronic health record systems; strong documentation and research capabilities.
- Skills & Attributes: Exceptional communication, leadership, and time management skills, with a proactive, detail-oriented approach to patient care and a strong commitment to interdisciplinary collaboration.
- Language Requirement: Bilingual in Spanish and English preferred.
