Description:
- Primary Care/Family Medicine Nurse Practitioner
- Monday - Friday schedule, rotating weekend coverage as needed
Responsibilities:
- Treating patients in the clinic with behavioral health issues and addictions
- Performing H&Ps, mental health assessments and a full spectrum of healthcare services (mainly to adult population)
- Develop and implement patient management plans
- Educate patients on overall wellness and health maintenance
- Maintain a safe and clean work environment
Qualifications:
- At least 1 year of experience preferred
- Active Connecticut License
