The Judge Group is looking for Nurse Practitioners and Physicians Assistants to complete Home Health Risk Assessments in Maricopa County, AZ.
Position Summary - The Nurse Practitioner (NP) provides comprehensive healthcare services to patients, impacting patient outcomes and promoting continuity of care through in-home and community-based visits.
Responsibilities
- Conduct comprehensive in-home patient assessments (minimum 25 visits/week).
- Assess patients' health status, diagnose, and develop treatment plans.
- Manage chronic diseases and medications.
- Promote preventive care and provide patient education.
- Collaborate with healthcare professionals for coordinated care.
- Serve as patients' primary care provider.
- Maintain accurate EMR within 48 hours of visits.
- Exhibit exceptional bedside manner.
- Schedule follow-up appointments.
- Participate in quality improvement and after-hours on-call.
- Ensure reliable transportation and comply with company policies and legal regulations.
Qualifications
- Active, unrestricted NP licensure in the state of practice.
- Certification as a Family or Adult-Gerontology NP.
- Obtain autonomous provider status within six months of eligibility.
- Preferred two years of experience in primary or longitudinal care.
- Active DEA license, BLS Certification, strong clinical skills.
- Excellent communication and EMR proficiency.
- Valid car insurance and driver's license with a clean record.
- Pass a comprehensive background check.
