Nurse Functional Manager
The Center for Elders' Independence is a PACE (Program of All-Inclusive Care for the elderly) organization that uses an interdisciplinary team approach to care planning and care implementation for the purpose of providing high quality, affordable, integrated health care services to the elderly, and promoting autonomy, quality of life and the ability of individuals to live in their communities.
The Position : Under the supervision of the Senior Director of Operational Excellence, the Nurs e Functional Manager is responsible for the clinical and administrative management of the PACE Clinics and Home Health Nursing, including initiation, planning, implementation, and improving nursing services with an emphasis on providing quality participant care. Supports the clinical operations in interviewing, hiring, and onboarding of nursing staff. This role provides clinical leadership, promotes standardization of nursing practices, and drives continuous improvement initiatives to enhance care outcomes and operational efficiency aligned with organizational performance and quality measures. The Nursing Functional Manager is responsible for the overall clinical functions focusing on effective and efficient operations of the clinic and home health services to provide value-based care in the PACE setting (center or home-based).
Compensation range for the Nurse Functional Manager role at CEI is $43.81 - $65.71 Offers based on level of experience/education as it applies to the requirements for the position Duties and Responsibilities:
- Responsible for teaching, coaching, training and assessment of clinical competencies of the clinic/nursing staff to continuously improve clinical, financial and service outcomes.
- Ensures clinic/Nursing staff provides the highest quality of care and in compliance with the California Board of Registered Nursing, Board of Vocational Nursing & Psychiatric Technicians, California Certifying Board for Medical Assistants, federal, state and local requirements.
- Leads process improvement efforts with Clinic Supervisors that support evidence-based practices and quality initiatives.
- Develops, implements, audits and communicates clinical practice, policies, and procedures for best practices, evidence-based practice standards, and provides input to Clinic Supervisors and Center Directors on the evaluation of clinic personnel at each PACE location.
- Utilizes quality improvement tools to collect, analyze, interpret, and display data that showed improved processes and patient care and reports during Quality Management meetings.
- Maintains own clinical awareness and competencies as required by the department to assure competency of staff and mentor staff to improve clinical performance and problem-solving skills.
- Collaborates with the Clinic Supervisor and Center Directors by responding and resolving patient care issues.
- Collaborates with other disciplines as needed and acts as a resource to Interdisciplinary teams across the program.
- Contribute to the recruitment, screening and hiring of clinic staff to ensure clinical competence, as needed by Clinic/Center leadership.
- Assesses and maintains a clinic staffing model that efficiently and effectively utilizes the appropriate resources ensuring competency, skill mix and adequate number of staff.
- Manage s clinical orientation and onboarding for clinic/home health nursing staff to ensure proper orientation and onboarding in a timely, appropriate, and comprehensive manner to meet all PACE program needs.
- Provides functional and technical Nursing consultation to Clinic Supervisors and Center Operations. Acts as subject matter expert in the development of performance improvement plans.
- Seeks out opportunities to build relationships with Functional Managers and other people resources who can contribute to the success of department's initiatives. Identifies opportunities to improve service delivery and/or program growth.
- Develop s , improve s , oversees training and monitor s clinical workflows for efficiency, quality and satisfaction.
• Acts as an administrative liaison and resource with administrative support departments and
assists with regulatory management, including compliance in all areas of PACE Nursing
services including tools, equipment and participant safety, performance against national
benchmarks and established service standards, and preparing the necessary Corrective
Action Plan (CAP) as needed and in a timely manner.
• Serves a leadership role in regulatory audits for both preparation and as a subject matter
expert to ensure adherence to regulatory requirements.
• Participates in developing, monitoring and revising department policies and procedures
consistent with current standards of practice.
• Determining clinic/Nursing tools/ supplies and equipment needs and specifications
consistent with program needs and budget in collaboration with Clinic/Center leadership.
• Performs other assigned duties, demonstrating flexibility and a positive proactive approach
to participant care and business needs.

- Proficient in using Electronic Health Records (EHR) software(s), Microsoft Office programs (Word, Excel, PowerPoint) and project management software .
- Knowledge of relevant regulations from agencies such as CACFP, OSHA, California Department of Healthcare Services (DCHS), Center for Medicare and Medicaid Services (CMS) - PACE Regulations.
- Maintains eligibility for licensure by obtaining required c ontinuing e ducation units .
- Ability to react calmly and effectively in emergency situations.
- Strong verbal and written communication skills to effectively interact with participants, families and team members.
- Ability to work as a team player in a multi-cultural, multi-disciplinary setting
- Must have a valid California driver's license , motor vehicle insurance and reliable transportation .
Qualifications:
- Graduate from an accredited school of professional nursing.
- Bachelor of Science in Nursing (BSN) required. Master's degree in healthcare related field preferred.
- C urrent California Board of Registered Nurse Licens e required.
- Current CPR - BLS certification required.
- Have a minimum of (1) year acute , skilled nursing facility or ambulatory care experience, working with the frail or elderly population required .
- Have a minimum of (1) year of leadership and/or project management experience working with cross-functional teams required .
