Provides case management to coordinate care into a comprehensive diabetes health care plan. Develops and revises programs for diabetic patients with other members of the diabetes team. MINIMUM QUALIFICATIONS: GS-11: Your resume must demonstrate at least one (1) year of professional nursing experience equivalent to at least the GS-09 grade level in the Federal service obtained in either the private or public sector, AND possess a diploma, associate degree, bachelor's degree, or master's degree in nursing from a professional nursing educational program. Professional nursing experience includes the following type of work and/or tasks: Planning and conducting comprehensive diabetes self-management and prevention education programs; independently develop care plans and provide education to patient with diabetes or at risk for diabetes; provide continuous professional nursing care, practices, and standards of care for patients with diabetes; analyzing and evaluating data and information to identify health needs and for program improvement. OR Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field. Time In Grade Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level.
