This part-time primary care role will include patient care rounding in a post-acute care setting in a skilled nursing facility (SNF) located in the Chesterton, Indiana, area.
About the RoleAs a weekend clinician in a skilled nursing facility environment, you will be an integral part of our care team, providing comprehensive primary care services to our acute patients. Your responsibilities will include:
- Conducting thorough patient assessments and developing individualized care plans
- Prescribing medications and treatments
- Collaborating with physicians and other healthcare professionals
- Providing education and support to patients and their families
Qualifications- NP/PA license
- Active and unrestricted medical license in Indiana
- Strong clinical assessment and diagnostic skills
- Excellent communication and interpersonal abilities
- Experience in post-acute care a plus
Expected compensation is estimated salary range of $27,450 to $50,450 annually with no cap on productivity income potential.
