POSITION SUMMARY/RESPONSIBILITIES
This position will serve as the link between the patient and family, community, volunteer staff health system providers, social workers and referring physicians in order to proactively prevent patient and/or family concerns or unmet needs escalating into a formal complaint and continue to build on our customer service The patient navigator of the University Transplant Center explains the process of receiving care in the department and recognizes and removes institutional barriers to the delivery of optimal customer service. The patient navigator proactively evaluates and resolves service issues.
EDUCATION/EXPERIENCE
Bachelor's degree in social sciences, health education or related field of study is required. Four years of experience in the area of health education, public health, social work and/or case management may be considered as a substitute in lieu of the Bachelor's degree. English/Spanish fluency is required.
