Job description
- Maintain accurate records in the Panoramic and Truelink systems to meet compliance requirements
- Provide ongoing case management and connect clients with community resources (e.g., medical, psychiatric, substance use, vocational)
- Participate in staff meetings, case conferences, and training sessions
- Provide representative payee services and create financial disbursement plans for clients
- Manage client budgets, ensuring bills and essential needs are covered
- Maintain up-to-date financial records in the Panoramic system
- Help clients with Social Security appeals, overpayment waivers, and utility discounts
- Monitor client accounts for unusual activity and notify relevant agencies of entitlement changes
- Submit accurate monthly reports and timesheets by required deadlines
- Report emergencies, work-related injuries, or breaches of privacy immediately
- Maintain professional boundaries with clients and collaborate with internal and external stakeholders
- Notify Program Director of absences or schedule changes in advance
Qualifications
- Experience working with adults with mental health, substance use, or other life challenges
- Familiarity with community resources, housing programs, and entitlement services
- Strong organizational and documentation skills; experience with case management software (e.g., Panoramic) preferred
- Knowledge of HIPAA regulations and mandated reporting requirements
- Ability to maintain professional boundaries and set appropriate limits
