We are searching for a Nurse Manager, Outpatient for TCP Westwood - someone to maintain accountability for clinical and personnel activities and to provide support to the Director, Medical Director and Practice Administrator for outpatient clinic operations. The Patient Care Manager participates in the delivery of high quality, family-centered and developmentally appropriate patient care at the clinic level. The Patient Care Manager provides leadership to direct clinic patient flow, staffing and clinical quality in collaboration with physician and administrative outpatient leaders.

Qualifications Required
  • Bachelor's degree in nursing
  • Current RN Licensure by the Texas Board of Nurses or Nursing Licensure Compact
  • BLS from AHA
  • 2yrs of nursing experience (4+yrs preferred)
  • 2yrs Leadership experience in an outpatient setting preferred
Specific Duties
  • Coordinates and collaborates outpatient clinical operations and patient care needs with appropriate members of the healthcare team to achieve desired clinical outcomes and management of care across the continuum.
  • Demonstrates effective communication skills with health care team and works to eliminate communication barriers.
  • Serves as a liaison between physicians, families, nursing staff and ancillary departments.
  • Observes, identifies, and addresses human resources issues for assigned staff.
  • Coaches, counsels and develops staff to ensure the delivery of high-quality patient care
  • Participates in clinic-based quality initiatives for the purpose of developing and monitoring clinical outcomes that demonstrate nursing excellence
  • Conducts annual learning needs assessment (in collaboration with the education coordinator) for direct reports and submits to Ambulatory leadership for annual professional training curriculum planning
  • Provides direct patient care to support outpatient clinical operations and patient flow
  • Maintains clinical competency by ensuring that the physiologic, psychosocial, and developmental needs of the patient are met within a Family-Centered care environment
