MINIMUM QUALIFICATIONS:

A) LPN with one year experience working in a Human Service field (ie. Long term care, OFA, DSS or Dr.'s office etc.); or

B) Graduation from a regionally accredited or New York State registered college or university with an Associate's Degree in human services with 2 years experience in a human service field as indicated above; or

C) Graduation from a regionally accredited or New York State registered college or university with a Bachelors Degree in Human Services; or

D) A combination of education and experience as described above.

TYPICAL WORK ACTIVITIES:

Functions as a member of the Mental Health team;

Assists the nurse in assessing patients mental status in psych education, admission interviews and emergency psychiatric situations, reports finding to physician and assists with disposition;

Observes and reports patient behavior and needs on the clinical unit;

Assists in planning individualized therapies for each patient assigned to him/her during staff conferences, physician rounds, and meetings pertinent to patient care;

Interacts on individual one-on-one basis with patients assigned to staff nurse;

Participates in group therapy sessions as observer or leader in areas of his/her expertise;

Assists Activities Therapist in their groups and substitutes for them in their absence with direction;

Accurately documents patient behavior, affect, cognition, and participation in unit activities;

Demonstrates and models desired behaviors such as objectivity, concern and empathy;

Determines patient's needs and arranges disposition or admission, including contact with other agencies and transfer to other psych facilities when appropriate;

Provides families with information regarding patient's rights, etc. keeping in mind confidentiality and reports problems to staff nurse;

Interacts supportively with the patient, their family and other members of the patient care team;

Maintains safety of patients and personnel by observing and reporting potential problems;

Aids in managing crisis situations;

Assists patient with activities of daily living as required;

Completes vital signs and reports changes in patient behavior to staff nurse
