Job summary The SCFTis a multi-disciplinary team supporting transition, and continuedrehabilitation for service users, out of secure care into the community. Theteam provides service users with care and treatment pathways which offerintervention and forensic risk management. This is a regional team and operatesacross West Yorkshire.

Healthy social and occupationalnetworks outside of secure care are an important function when managing risk ina community setting. The post holder will hold a small individual caseload, havethe opportunity to complete identified Occupational therapy assessments toaddress the occupational deficits of service users and support the wider teamin formulating care, to ensure our service users have opportunities for socialnetworks and activity, and thrive in the community. The post holder will alsotake on role of care coordinator.

Thisrole includes working shifts including long days and mid-shifts that cover a 7-dayservice within the hours of 08:00-20:00pm. Access to a vehicleis essential.

Main duties of the job TheAll-Age Forensic Community Service is a developing service line within theForensic Care Group. It also includes:

ForensicCAMHS (inclusive of Y&H FCAMHS, Focus Secure and Focus Wakefield)

ForensicOutreach and Liaison Service for Learning Disability and/or Autism (FOLS LDA)

Our adultforensic community service (Forensic Community Transition Team (Bretton Centre)and the Specialist Community Forensic Team (SCFT

Thevision is to create a collaborative environment between our community forensicservices and to develop and overarching clinical framework that will supportthe way in which a community forensic population is understood, supported andintervened upon, no matter what age. The purpose of the role is therefore to:

Be the lead professionalin the case management of service users.

Undertake comprehensiveassessments & risk assessments

Be enthusiastic aboutthis service user group and motivated to support change and transition needs ofindividuals.

Able to demonstrate theability to work autonomously and collaboratively with other agencies to makeclinical judgements promoting joint decision making about care delivery toindividuals with complex presentations.

Bethoughtful and resilient in the way you engage with a multidisciplinary team.

Have strongand effective engagement skills to service users, family, friends and carers.

Person Specification Qualifications
•  RMN Current NMC registration.
•  Social Worker/Occupational Therapist-current HCPC registration.
•  Experience of mentoring/supporting learners or ability to demonstrate the skills required for effective mentoring.
•  Practice Education/Practice Assessor Preparation Certificate.
•  Occupational Therapist
•  Diploma/degree in Occupational Therapy.
•  Health Care Professions Council registration.
•  Occupational Therapist
•  Leadership / first line management certificate. Training
•  Assessment and care planning skills.
•  Risk assessment and risk management.
•  Relevant post-registration qualifications and / or training.
•  Highly developed communication and interpersonal skills.

•  Practice Education/Practice Assessor Preparation Certificate (or to achieve within 12 months).
•  To be willing to undertake post graduate training to further skills, knowledge and expertise.
•  Successful completion of venepuncture course and able to demonstrate competence.
