Job Description:

We have a wonderful Full-Time opportunity to join our Evoke Wellness at Hilliard team as a Licensed Assessment Clinician.

The primary function of this position is to provide client diagnosis, presenting problems, and treatment/recovery goals. The clinician is also responsible for evaluating the clients' level of readiness and motivation to engage in treatment.

Job Responsibilities
Provides behavioral health screenings and assessments with clients
Performs safety assessments
Provides treatment planning in coordination with the client, family, and other key team members.
Documents assessments and treatment plans timely, clearly, and concisely in the electronic health record per Evoke standards that comply with all accrediting regulatory agencies
Maintains all records and patient information in a confidential manner according to Evoke standards.
Collaborates and coordinates with internal Evoke departments to assure appropriate services are utilized for optimum client care.
Actively participates in staff trainings/meetings.

Knowledge of EMR system, Microsoft Office Programs (i.e. Excel, Word, Outlook, etc)

License/Certification: Licensure to practice clinical discipline in the appropriate state, as applicable, required in Social Work, Marriage and Family Therapy, and Mental Health Counseling in the State of Ohio.

Education/Work Experience :
Master's degree in the mental health field from an accredited institution.
Experience in mental health and addiction environments
