The Care Homes Assessment Team (CHAT) is an award-winning integrated physical and mental health service supporting residents in Care Homes across Enfield, part of Haringey. CHAT, within Enfield Community Services (ECS) hosted by NMUH, includes dedicated Consultant Psychiatry support, a mental health band 7 RMN, and OT staff. We seek a flexible, experienced band 7 Mental Health Specialist Nurse to join us during maternity leave. The ideal candidate will possess assessment and diagnosis skills, be willing to undertake independent prescribing if not already qualified, and have experience working with dementia or older adults with mental health or learning disabilities. Experience in sensitive care planning and end-of-life support is essential. The service operates 7 days a week, 8am-8pm, with shift patterns of 8am-4pm or 9am-5pm.

Main duties of the job

To assess patients systematically, analyze signs and symptoms, formulate diagnoses, and make referrals for residents in Care Homes. To develop evidence-based, individualized care and treatment plans in collaboration with patients, their relatives, and Care Home staff, considering the presentation of psychological illnesses in the elderly. To establish appropriate interventions and contingency plans, including end-of-life care, to prevent unnecessary hospital visits and improve end-of-life support. To support an unplanned care telephone triage service, providing advice and organizing assessments. To review medications in consultation with GPs or psychiatrists, especially regarding polypharmacy. To promote safety, report safeguarding concerns, and participate in safeguarding meetings to protect vulnerable residents.

Person Specification Education & Professional Qualifications
  • Registered Mental Health Nurse with current NMC registration
  • CPD in psychological condition management
  • Degree in a psychological health-related topic
  • Community Specialist Practice
  • Masters Level Education
  • Postgraduate training in Dementia and/or Delirium management
Skills and Aptitudes
  • Manage complex psychological illnesses within a caseload
  • Work proactively with A&E, ERAS, GPs, and Social Care to prevent hospital admissions
  • Assess and plan care with patients and carers

Salary: £51,883 to £58,544 per annum, including outer London HCAs
