Laurel Parc at Bethany Village, a beautiful independent senior living community located just outside of downtown Portland near Beaverton, seeks an experienced and effective Memory Care Director to provide overall management and support functions of the Memory Care team, as well as maintaining regulatory compliance and ensuring resident care systems follow Laurel Parc standards of service, policies and State requirements.
Duties & Responsibilities
- Conducts resident care conferences timely and notifies responsible party of changes in the plan of services as directed by Health Services Director.
- Reviews schedule for medication assistants and caregivers, including taking calls 24 hours a day, seven days a week, alongside Resident Care Coordinator and Staffing Coordinator.
- Tracks attendance and overtime issues.
- Reviews all physician orders and audits to ensure transcriptions are accurate.
- Conducts new move in evaluations and readmit evaluations.
Other Job Duties
- Ensures staff complete all necessary pre-service training and continuing education per state regulations and Laurel Parc policy.
- Participates in the hiring, termination and coaching process for employees.
- Provides direction to the department team members to assure residents are provided the highest quality care.
- Orders inventory supplies for department.
- Follows up on all consultant reports timely to include but not be limited to; pharmacy, state licensing, contracted consultants, etc.
- Reviews monthly incident summary reports for accuracy and evaluate trends.
- Assists Executive Director with projects that are requested.
- Assures medications are ordered timely and properly documented per physician orders.
- Coordinates and communicates resident care needs with residents, family and outside providers and assures recommendations are carried out.
- Maintains all privacy rules and regulations pertaining to the Health Insurance Portability and Accountability Act (HIPAA).
- Works collaboratively in a team environment with a spirit of cooperation.
- Displays excellent communication including the ability to communicate effectively and remain calm and courteous under pressure.
Education & Experience
High school diploma or general education degree (GED) + 3 years leadership responsibility working with the senior population in Memory Care or Assisted Living required.
