- Schedule: Monday - Friday; NO major Holidays, and NO on-call
- Shift: 5x8's Days
Travel vs. Local:
- Travel Contract which requires the LMSW, LGSW, or LCSW to live 51 miles outside of "Location" (see above) to qualify for Stipend
Travel Pay Package:
- Untaxed Travel Stipend: $1,600
- Weekly Gross: $2,200 - $2,280
Skill Required:
- Discharge Planner Case Manager
- Shifts: (8A-430P) 5 x 8 Days
- On call/Weekend Requirement: no on call requirement; weekends included in rotation (every 5th or 6th weekend); holiday coverage as needed
- Floating Requirement: no floating requirement
- Local Rate within 60 miles of facility
Required Experience/Licensure:
- Will accept RN Case Manager Discharge Planner or Licensed Social Worker - Social worker must be eligible for LMSW/LGSW/LCSW -they must have taken and passed the exam already
- DC RN Licensure -. WILL ACCEPT PENDING DC LINCEUSNRE WITH PROOF OF APPLICATION (Aya paid open license project) OR candidate can hold LMSW/LGSW/LCSW
- Must have acute hospital discharge planning experience
- Skills required: Emergency Department, Case management
