As a Medical Social Worker, you will:
Conduct initial evaluations and ongoing assessments of patients' social work needs. Assist with funeral planning, legal, and healthcare decisions. Provide education on advanced directives and community resources. Offer counseling and bereavement support to patients and families. Qualifications include a Master's degree in Social Work, state licensing, and at least one year of healthcare experience. Hospice experience is preferred.
