Medical Social Worker - Home Health
Location: Corvallis, OR (territory includes surrounding areas, within a 60-mile radius)
Compensation: $40-$50 per hour (commensurate with experience)
Travel Required: Yes (valid driver's license, personal insurance, and reliable vehicle required)

Position Summary
As a Medical Social Worker - Home Health, you will play a vital role in improving the emotional, psychological, and environmental well-being of patients receiving in-home clinical care. This position offers the opportunity to form meaningful patient relationships while providing life-enhancing support to individuals and families navigating health challenges.
Key Responsibilities
Conduct thorough psychosocial assessments to identify patient and family needs
Develop individualized plans of care tailored to social, emotional, and environmental conditions
Offer practical assistance through information and referrals to community and social support resources
Facilitate access to healthcare services by collaborating with caregivers, providers, and agencies
Maintain timely, accurate, and confidential clinical documentation
Serve as a supportive liaison between patients, their families, and broader care networks
Support discharge planning and care transitions in alignment with clinical goals
Required Qualifications
Master's degree in Social Work from a program accredited by the Council on Social Work Education
Active Licensed Clinical Social Worker (LCSW) credential (Oregon)
Minimum of 1 year of experience in a healthcare setting (home health preferred)
Current CPR certification
Valid driver's license and a reliable, insured vehicle
Strong communication, organizational, and case management skills
Preferred Qualifications
Prior experience in home health or community-based care delivery
Familiarity with public assistance programs and local support agencies
Ability to work independently while managing a mobile patient caseload
Physical & Travel Requirements
Frequent driving across a regional territory (up to 60 miles radius)
Physical ability to navigate home environments and engage in community-based service delivery
Must be comfortable working in varied environmental conditions (weather, home settings, etc.)
