Qualifications:
- board certified/board eligible in family medicine (FM) or internal medicine (IM)
- have a passion to serve active older adults

As a part of our team you will work with each patient's care team to implement an individualized plan of care that meets the patient's goals. We provide you with template progress notes that include HPI, current medications, general exam, and review of systems, assessments and treatments by diagnosis to allow you to manage your time and have work/life balance.
