Methodist Fremont Hospital-Dunklau Gardens-MDS Specialist RN-Full Time-$20,000 Sign On Bonus


Methodist Fremont Health-Dunklau Gardens is looking for a MDS Specialist RN to join our team. This role is responsible for ensuring the timely completion of accurate assessments and interdisciplinary care plans in accordance with federal and state guidelines. Responsibilities include identifying resident acuity and care needs, communicating with families and responsible parties, overseeing resident care, managing departmental budgets, and ensuring compliance with Resident Assessment Instrument (RAI) processes.


Job Duties and Responsibilities:

  • Completes and manages MDS assessments in accordance with RAI Manual, OBRA, and PPS guidelines, ensuring regulatory compliance and timely submission.
  • Assesses resident conditions and identifies care needs by evaluating acuity, diagnoses, functional status, and goals using interdisciplinary input.
  • Develops, implements, and evaluates individualized care plans, adjusting based on resident outcomes, progress, or decline.
  • Collaborates and communicates with interdisciplinary teams, residents, and families to ensure accurate care planning and shared understanding of goals and interventions.
  • Ensures accuracy of MDS data and coding for proper billing, compliance, and quality resident outcomes, including review of validation reports for errors.

  • Methodist Fremont Health - Dunklau Gardens: 450 E 23rd St. - Fremont, NE

Schedule:

  • Monday through Friday 8:00 a.m. to 4:30 p.m. routinely. Flexibility appreciated.

Job Requirements:

  • Graduated from an accredited Diploma School of Nursing/College of Nursing with an Associates of Science Degree in Nursing (ASN) or a Bachelor of Science in Nursing (BSN) required.

Experience:

  • Three years of experience in Long Term Care (LTC) required.
  • Three to Five years previous MDS experience preferred.

License/Certifications:

  • Current valid Registered Nurse (RN) license, valid compact multistate license, or a temporary permit while awaiting licensure required.
  • Current American Heart Association Basic Life Support (BLS) strongly preferred at time of hire, required within 3 months of hire.
  • MDS (Minimum Data Set) Coordinator Certification preferred.
