Job Summary
We are seeking a dedicated and compassionate Nurse to join our healthcare team. This role requires strong clinical skills, attention to detail, and the ability to work collaboratively with other healthcare professionals to ensure optimal patient outcomes.
Basic Qualifications Education and Certification: RN/LPN degree from an accredited college or nursing school; current RN/ LPN licensure within the state of practice.
Skills & Requirements: Must possess the ability to read, write and speak the English language; must be involved in health care matters and projects with the community as needed; must have strong communication and presentation skills in both group and individual situations; must be patient, friendly, courteous and respectful to residents in any situation, including difficult ones; must be able to make independent decisions when necessary; must have the ability to follow nursing department guidelines as outlined by the facility and other regulations governing nursing services in a long-term care facility; must be able to move frequently throughout the workday; must be able to lift 40 lbs floor to waist; lift 20 lbs waist to shoulder; lift and carry 40 lbs; and push/pull 40 lbs.
Experience:
Minimum two years nursing experience in long-term care or other health care setting; supervisory experience preferred.
Essential Job Duties
1. Develops, conducts and completes resident assessments timely, thoroughly and accurately.
2. Makes necessary revisions to the MDS as warranted.
3. Determines and creates a schedule for timely completion of periodic resident assessments and leads the coordination with the interdisciplinary team to follow the schedule.
4. Ensures completion of the initial resident assessment within timeframes upon admission as required by state and federal laws and regulations.
5. Oversees the the completion of quarterly and annual resident assessment and ensures that completion is timely and accurate.
6. Oversees and ensures the completion of a resident assessment within timeframes required by state and federal laws and regulations upon a significant change in resident's condition.
7. Educates, trains and directs staff on the requirements of timely and accurate completion of the MDS and supporting documentation.
8. Monitors assessments to ensure that all documents have been properly signed and dated.
9. Reviews and approves assessments to certify accurate and timely completion.
10. Garners feedback from other department managers and supervisors regarding resident assessment functions of the facility. Evaluate the efficiency and effectiveness of MDS procedures at the facility. Formulate and recommend any processing changes. 11. Partners with the DON to create and implement procedures affecting MDS procedures, including notification of all MDS staff of the admission of new residents.
12. Participates in facility and department orientation program for new employees, where it relates to MDS information.
13. Trains and educates nursing department on MDS process.
14. Perform medication administration duties in accordance with the care plan as ordered by a physician.
15. Promotes continuity of care through partnering with other departments (i.e. Rehab).
16. Participates in discharge planning process.
17. Participates in state or federal survey processes.
