MDS Nurse/ Coordinator
We are seeking a Part Time MDS Coordinator to join our exciting team. Full Time hours are available as this position could be supplemented with some hours on the floor.
The primary purpose of this position is to provide management of Resident care as related to the MDS and Care Plan and in accordance with local, state, and federal regulations and guidelines, and the CareCore Healthcare philosophy and as directed by the Director of Nursing and Administrator. As the MDS/Care Plan Coordinator, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.
This position is responsible for the following:
Ensure that the facility is compliant with the guidelines for the Minimum Data Set (MDS)
Medicaid and Medicare RUGs/PDPM
Medicaid Case Mix
Medicare Criteria including the supportive documentation requirements
Including knowledge of Quality Measures and Five Star ratings criteria.
The successful candidate will possess, at minimum , the following qualifications:
Licensed Nurse in the State of Ohio or eligible for licensure in Ohio
1+ years of experience as a MDS Coordinator or equivalent experience.
