We're looking for a dedicated RN MDS Coordinator to lead the MDS assessment process and support quality care planning at The Pearl Nursing Center of Rochester.

What You'll Do:

Oversee and complete MDS assessments accurately and on time

Collaborate with our care team to develop resident care plans

Ensure compliance with state and federal MDS regulations

What You Need:

Active NYS RN license

MDS experience in long-term care

Strong attention to detail and teamwork
