MDS Coordinator job responsibilities include;

Knowledge of all aspects of the RAI/MDS process to include person-centered care planning, Medicare PDPM reimbursement, scheduling, month-end triple check, appeals process and Medicare certification process

Maintain current PDPM/Medicare and Medicaid/Private assessment schedules with proper reference dates for regular distribution to therapies, dietary, social service, activities and nursing.

Provide ongoing education and training for MDS and person-centered care planning

Understand and follow through with the RAI process, including MDS assessment, Triggers and CAAS, and Care Planning.

Maintain the Medicare certification/ recertification binder to meet the requirements of Medicare physician certification for Part A & B services.

Demonstrate current knowledge of nursing practices and procedures as they pertain to Long Term Care including state, federal, and OBRA regulations.

Requirements:

Registered Nurse (RN) with a valid New Hampshire license

RAC-CT certification preferred

Minimum 2 years' experience in long term care, including experience with the RAI process and person-centered care planning

Must have strong clinical and communication skills

Extensive knowledge of the RAI process and reimbursement requirements for both federal and state

Must be highly organized with the ability to multi-task throughout the day

Experience with Microsoft PowerPoint, Word, Excel and Outlook preferred

Experience in developing templates to utilize in facility for audit/educational use
