The MDS / Care Plan Coordinator RN or LPN ensures timely and accurate resident assessments and interdisciplinary care plans in compliance with Federal and State guidelines. Responsibilities include:
Integrating nursing, dietary, social, and rehabilitation services for proper assessment and reimbursement. Completing Medicare/Medicaid case-mix documents for appropriate reimbursement. Conducting ongoing MDS reviews to achieve optimal Resource Utilization Group (RUG) categories. Overseeing MDS/PPS documentation and submission processes. Preferred qualifications: Current RN licensure, long-term care experience, and proficiency in English.
