Job Summary: The MDS / Care Plan Coordinator RN or LPN ensures timely and accurate resident assessments and interdisciplinary care plans in compliance with Federal and State guidelines. Responsibilities include identifying resident needs, integrating various services, and managing Medicare/Medicaid documentation for reimbursement.
Essential Duties:
Integrate nursing, dietary, social, and rehabilitation services for assessments. Complete Medicare/Medicaid case-mix documents for reimbursement. Conduct ongoing MDS reviews for optimal Resource Utilization Group (RUG) categorization. Oversee MDS/PPS documentation and submission processes. Benefits Offered:
