MDS Coordinator

•  Full-time dayshift MDS Coordinator position with a $12k sign-on bonus for full time ($6k for part time); must be an RN or LPN.
•  Key duties: Lead timely and accurate MDS assessments, supervise data entry, ensure regulatory compliance, present reports to DON, assist with audits/surveys, and support quality improvement.
•  Requirements: Accredited nursing graduate (RN, BSN, or LPN), active RN license, 3+ years clinical long-term care experience, prior MDS/RAI experience, and knowledge of Medicare/Medicaid.
