Job Title: MDS / Care Plan Nurse
Sign-On Bonus: $7,500
Summary: The MDS / Care Plan Nurse ensures timely and accurate resident assessments and interdisciplinary care plans in compliance with Federal and State guidelines. Responsibilities include assessing resident needs, coordinating care services, and managing Medicare/Medicaid documentation for reimbursement.
Essential Duties:
Integrate nursing, dietary, and other services for assessments. Complete Medicare/Medicaid case-mix documents. Conduct MDS reviews for optimal RUG category. Oversee MDS/PPS documentation and submission. Benefits:
