Job Summary:
Under the supervision of the Director of Population Health, the Manager of Population Health Care Coordination is responsible for the implementation and delivery of the Population Health Care Coordination program for Loyola Physician Partners. The Manager provides oversight and daily management of the Nurse Care Coordinators, Community Health Workers, Social Workers and other population health staff. They ensure staff activities are focused on achieving the Population Health program goals to improve quality of care, mitigate cost trends through supporting optimization of care delivery and improve member and physician satisfaction. They oversee delivery of an integrated care management program based on the individualized needs of the patient to ensure appropriate care at the right time and in the right setting in order to improve clinical outcomes and avoid adverse outcomes. This individual directs the use of Risk Stratification and pertinent clinical information through Comprehensive Health Assessments, Care Plan Development, Transition/Discharge Planning Management from each setting of care, Complex Case Management, Readmission Prevention and Establishment and Maintenance of Relationships with Post-Acute Providers. The manager provides guidance to staff on day- to- day operational issues, assesses staff performance using productivity and clinical outcome measures, and supports the Director in evaluating program effectiveness and efficiency.
The Manager of Population Health Care Coordination supports the program goals of ensuring clinic, ED, hospital and post-acute system resources are used appropriately and effectively. They review utilization and quality data to monitor performance of care coordinator staff. They identify individual staff and overall programmatic opportunity areas and work with LPP leadership to develop strategies to address performance opportunities.
Requirements:
- Bachelor's degree in Nursing; Master's degree preferred.
- Current RN License in IL
- 6-10 years of previous job-related experience
- Utilization management, case management, discharge planning or other cost/quality management program.
- 3-5 years of experience in hospital-based setting and experience in community-based nursing or social services preferred.
- A minimum of 3-5 years of management experience preferred.
Compensation:
Pay range: $75,129 - 120,224
Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
