We are seeking an experienced and motivated Manager of Case Management to lead a high-performing team in a busy acute care setting. This full-time leadership role is ideal for a Registered Nurse with strong clinical expertise and a background in case management or nursing leadership.
Position Overview
The Manager of Case Management is responsible for the day-to-day operations of the Case Management department. This includes oversight of staffing, workflows, and patient throughput processes to ensure efficient, high-quality care and discharge planning. The role reports directly to the Director of Case Management & Social Services.
Key Responsibilities
- Support case management staff through scheduling, workload balancing, and rounding on clinical units.
- Oversee timely and appropriate discharge planning to ensure continuity of care.
- Develop strong relationships with physicians, payers, and care teams to support effective care coordination.
- Ensure departmental compliance with hospital policies and regulatory requirements.
- Provide coaching, training, and ongoing support to team members.
Qualifications
- Bachelor of Science in Nursing (BSN) required; Master's degree (MSN) preferred.
- Current California Registered Nurse (RN) license.
- Minimum of 5 years of experience in acute care case management and/or nursing management.
- Strong knowledge of federal and non-federal case management regulations.
- Familiarity with InterQual and M&R criteria.
- Understanding of hospital reimbursement models and payer systems, including Medicare, Medi-Cal, managed care, and private insurance.
Compensation
- Salary range: $142,771 - $214,032 annually
- Relocation assistance available on a case-by-case basis
