Education Required: Bachelor's degree in Nursing (or actively enrolled in a BSN program) or Bachelor's degree in Social Work. Essential Duties and Responsibilities: Provide exceptional service and/or care to all patients, customer and team members. Carries out supervisory responsibilities in accordance with the Hospital's/Clinic's policies, procedures, applicable laws and regulations. Reviews departmental processes and procedures, assists in the development of or revision of needed processes or procedures. Provides work instruction and assists employees with difficult and/or unusual assignments. Assigns and distributes work, reviews work for accuracy and completeness, and returns assignments with recommendations for proper completion. Identifies problems, determines accuracy and relevance information, and uses sound judgment to generate and evaluate alternatives and to make recommendations for solutions. Manages and resolves conflicts, grievances, confrontations, or disagreements in a constructive manner to minimize negative personal impact. Responds orally to informal grievances and relays information to the Director. Abstracts data from records and maintains statistics. Determines patient review dates according to established diagnostic criteria. Assists review committee in planning and holding federally mandated quality assurance reviews. Responsible for providing assistance with managing team workloads by coordinating and participating in various aspects of Case Management services. This includes utilization management/review, referrals to outside agencies, eligibility determination, monitoring, assessment, and evaluation of patient needs / services. Monitors work activity on caseloads to ensure that cases are clearly and thoroughly documented. Maintains a thorough and updated knowledge of community resources, nursing facilities and agencies, Interqual and Milliman Screening Criteria. Provides staff updates with changes in rules and regulations. Interprets and explains state and federal rules and regulations to staff. Analyzes insurance, governmental, and accrediting agency standards to determine criteria concerning admissions, treatment, and length of stay of patients. Reviews application for patient admission and approves admission or refers case to facility utilization review committee for review and course of action when case fails to meet admission standards. Compares inpatient medical records to established criteria and confers with medical and nursing personnel and other professional staff to determine legitimacy of treatment and length of stay.
