Job Description:
A leading healthcare provider is seeking a highly motivated and experienced RN Manager of Case Management to oversee case management operations in a dynamic, full-service acute care facility. This leadership role is responsible for coordinating an interdisciplinary team to deliver efficient, patient-centered care, ensuring patients receive appropriate discharge planning and resource utilization during their hospital stay.
The RN Manager will lead a team of case managers, social workers, and support staff, promoting clinical excellence, regulatory compliance, and a seamless continuum of care. The ideal candidate will possess strong leadership abilities, outstanding communication skills, and a commitment to improving patient outcomes through collaboration and evidence-based practices.
Key Responsibilities:
- Facilitate effective patient care management and comprehensive discharge planning in collaboration with healthcare teams.
- Conduct face-to-face assessments with patients and families to evaluate discharge needs, functional capacities, and support systems.
- Perform timely utilization reviews to ensure appropriate use of hospital resources and efficient patient throughput.
- Communicate clinical information to third-party payers to support admission and continued stay requirements.
- Maintain active collaboration with physicians to address treatment criteria, optimize care delivery settings, and manage lengths of stay.
- Advocate for patient rights and ensure patient-centered care planning that promotes independence and continuity of care.
- Act as a liaison between patients, families, physicians, staff, and community service providers to facilitate communication and service delivery.
- Oversee community referrals, durable medical equipment coordination, home health services, and placement arrangements as needed.
- Lead quality improvement initiatives, ensure HIPAA and regulatory compliance, and contribute to operational efficiency.
- Mentor and supervise case management staff, promoting professional development and fostering a culture of teamwork and excellence.
Qualifications:
- Current, active Registered Nurse (RN) license.
- Bachelor's Degree in Nursing (BSN) required.
- Minimum of 2+ years of supervisory or management experience in a case management unit within an acute care hospital setting.
- Strong knowledge of discharge planning, utilization review, insurance authorization processes, and community resources.
- Proven leadership skills with the ability to inspire and drive team performance in a fast-paced healthcare environment.
- Exceptional communication, critical thinking, and organizational abilities.
Additional Information:
- Oversees a team of approximately 18 full-time employees, including case managers, social workers, and a case management coordinator.
- Reports directly to the Director of Case Management.
